GQR Healthcare-Allied is seeking a travel Physical Therapist for a travel job in Thousand Oaks, California.

Candidate Requirements
Minimum Experience Required: 2 years of Home Health experience preferred, wants home health experience that’s recent so there is less training required, OASIS experience is a must, HCHB experience is a strong strong preference
Travel Requirements: N/A
License/Certifications:  CA RN, BLS (not a compact state)

Assignment Details
Start Dates: ASAP
Scheduling: Monday – Friday, 40 hours
Shift/Hours: 8 am – 5 pm
On Call:  N/A

Additional Facility Notes

• 25 point system, lots of start of care visits, 2.5 for start of care, lots of start of care visits, 6-7 per day depending on visit
• Wednesdays they do team meetings and can be done remotely
• They create a Teams group chat for every patient, a way for RNs, Ots, PTs, PTAs, and directors to all communicate together on the same patient
• They work with PTAs for regular visits
• 32 hours guaranteed, really would like 40 max
• Travel reimbursement to each patient location (.70 cents per mile)
• Reason for opening – growth and another out on workers comp
• Territory covers entire Ventura county, large county so they will try to get visits close together to avoid long drive times between locations
• Culture: client wants someone who can get up and running almost immediately, their expectation is if they are paying a premium then they need someone experienced, will do orientation like their standard perm hires, if traveler needs more training then bill rate will drop to $85 per hour for the extra time
• Teams is huge for them, main way of communication, people typically stop responding once their work is done with the patient, like to shut their phones down at the end of the day and keep work and personal separate, everything can be answered the next day mentality if after hours
